Before your visit
To schedule an appointment, call the office you'd like to visit and our team will find you a time. There are no forms to fill in beforehand — bring the items below and we'll take care of the paperwork when you arrive.
What to bring
- Photo ID
- Insurance card, if you have one
- List of any medications

We accept most major dental insurance plans, including:

Plans change. Call either office with your plan details and we will confirm your coverage before your visit.
